Ask your Reliable Mortgage Advisor which pertain to your application. Personal information Driver’s License Social Security number Insurance agent name and phone number Residence with complete addresses including zip code If tenant, name and address of landlord If owned home, name and address mortgage company with balances If using child support, alimony or separate maintenance for qualifying purposes, divorce decree and/or separation agreements If no established credit history, supply cancelled checks for rent, utilities, and other recurring obligations to show payment history and amount of revolving debt. If VA Loan, Certificate of Eligibility and form DD214 Earnings Statements W-2 forms (2 years) Last 2 recent pay stubs (within 30 days) If self-employed or receive commission income, Federal tax returns (2 years) with all schedules included, for personal and business. Sign and date copies with original signatures and current date signed. Additional Income Most recent statements for bank accounts, mutual funds, retirement accounts and any other assets (internet statements not acceptable) Social security, veteran’s benefits, etc. will require a current award letter with proof that income with continue for three years Interest income, rents, etc. should be discussed with your mortgage advisor Any gift letters if using gift towards down payment or closing costs Employment Name and address of employers (2 years) Letter of explanation for any gaps in employment Assets List of all assets with estimate of value; if automobile is paid off, the free and clear title List of bank account numbers and bank contact information Copy of Settlement Statement on any previously owned homes If obtaining down payment from sale of previous home, copy of closing statement and estimated net proceeds Copies of deeds and notes on owned property and lease agreements on rental properties Debts Creditor account numbers and contact information, approximate balances and monthly payments (including credit cards) Statements for all mortgages or deeds of trust securing real property Complete copy of any bankruptcy papers, including discharge Alimony or child support payments
